Asuhan Keperawatan Pada Ny. Y Post Partum Dengan Persalinan Spontan Indikasi Ketuban Pecah Dini Diruang Kenari Rumah Sakit Daerah Kalisat Kabupaten Jember

ABSTRAK
Latar Belakang : Ketuban pecah dini apabila terjadi pada usia kehamilan lebih dari 36 minggu dan belum ada tanda-tanda persalinan maka dilakukan persalinan induksi. Pada kasus tertentu bila induksi partus gagal, maka dilakukan tindakan operasi sesaria.
Tujuan : Untuk mengetahui asuhan keperawatan pada pasien dengan post partum spontan dengan indikasi ketuban pecah dini meliputi pengkajian, diagnosa, intervensi, implementasi, dan evaluasi keperawatan.
Hasil : Setelah dilakukan tindakan keperawatan selama 3x24 jam didapatkan hasil nyeri pasien berkurang dari skala 7 menjadi 2, konstipasi sudah teratasi dan tidak terjadi adanya tanda-tanda infeksi.
Kesimpulan : Kerjasama antar tim kesehatan dan pasien/keluarga sangat diperlukan untuk keberhasilan asuhan keperawatan pada pasien, komunikasi terapeutik dapat mendorong pasien lebih kooperatif, teknik relaksasi progesif dapat mengurangi rasa nyeri dan merupakan tindakan yang disukai pasien. Makanan tinggi serat sangat dibutuhkan untuk mencegah konstipasi, kebersihan individu dan perawatan diri dapat mencegah infeksi.

ABSTRACT
Background: premature rupture occurs when the gestational age of more than 36 weeks and no signs of labor induction of labor is carried out. In the particular case when the induction of parturition fails, then performed a cesarean surgery. Objective: To determine nursing care in patients with post partum spontaneous premature rupture indications include assessment, diagnosis, intervention, implementation, and evaluation of nursing.
Results: After 3x24 hour nursing actions for pain patients showed reduced from 7 to 2 scale, constipation is resolved and there is no signs of infection. Conclusion: Cooperation between the healthcare team and the patient / family is indispensable for the success of nursing care to the patient, the patient's therapeutic communication can encourage more cooperative, progressive relaxation techniques can reduce pain and is the preferred measure patient. Foods high in fiber are needed to prevent constipation, individual hygiene and self-care can prevent infection.
