Abstract
Abstrak Tujuan umum diadakannya penelitian ini untuk mempelajari dan menerapkan konsep teori maupun praktek terhadap ashuna keperawatan pada Tn. F dengan kasus Post Operasi Hernia Skrotalis di Ruang Mawar RSD Balung Kabupaten Jember. Sedangkan metode kajian yang digunakan adalah pendekatan proses keperawatan yang terdiri dari pengkajan, diagnosa keperawatan, implementasi keperawatan dan evaluasi dengan pengambilan data dilakukan dengan metode yang berbentuk studi kasus asuhan keperawatan dengan kasus post operasi hernia skrotalis di Ruang Mawar Rumah Sakit Daerah Balung Jember. Berdasarkan pembahasan dapat ditarik kesimpulan bahwa dalam penerapkan asuhan keperawatan kepada anak dengan diare di Ruang Mawar RSD Balung Kabupaten adalah sebagai berikut: Pertama, Pengkajian: Pengkajian yang telah dilakukan tepat dapat mempermudah untuk mengetahui masalah keperawatan sesuai dengan keadaan dan kondisi klien agar mamapu menerapkan asuhan keperawatan pada klien dengan hernia scrotalis. Kedua, Diagnosa Keperawatan: Hasil pengkajian yang telah dilakukan oleh penulis pada Tn.F dengan post operasi hernia scrotalis didapatkan nyeri akut berhubungan dengan agen cidera fisik ditandai dengan post operasi herniaektomi. Resiko tinggi infeksi berhubungan dengan prosedur invasive ditandai dengan tindakan pembedahan. Gangguan mobilitas fisik berhubungan dengan nyeri ditandai dengan rentang gerak pasien menurun. Ketiga, Intervensi Keperawatan: Hasil analisa data yang didapatkan dari hasil pengkajian pada Tn.F dengan kasus post operasi hernia skrotalis penulis telah merencanakan tindakan keperawatan. Rencana tindakan yang disusun telah disesuaikan dengan masing masing masalah yang dialami oleh klien. Fokus pada bagian intervensi ini disesuaikan dari keadaan klien dan berdasarkan dengan diagnosa prioritas. Keempat, Implementasi Keperawatan: Implementasi keperawatan yang dilakukan oleh penulis kepada klien juga membina hubungan saling percaya terhadap keluarga klien supaya lebih memudahkan dalam pelaksanaan rencana tindakan. Kelima, Evaluasi: Langkah terakhir merupakan tahap evaluasi yang telah dilaksanakan secara optimal dan sistematis. Evaluasi yang telah dilakukan oleh penulis mulai tanggal 29 Oktober sampai 31 Oktober dari evaluasi ke 3 masalah yang didapatkan penulis dapat teratasi sesuai dengan kriteria hasil masing masing masalah keperawatan. Kata Kunci: Asuhan, Keperawatan, dan Hernia Skrotalis Abstrack The general purpose of this research is to study and apply theoretical and practical concepts to nursing asuna at Mr. F with the case of Post Scrotal Hernia Operation in the Mawar Room of the Balung Hospital, Jember Regency. While the study method used is a nursing process approach which consists of assessment, nursing diagnoses, nursing implementation and evaluation with data collection carried out by a method in the form of a nursing care case study with a postoperative scrotal hernia case in the Mawar Room of the Balung Jember Regional Hospital. Based on the discussion, it can be concluded that in applying nursing care to children with diarrhea in the Mawar Room of RSD Balung Regency, it is as follows: First, Assessment: Assessments that have been carried out properly can make it easier to find out nursing problems according to the client's circumstances and conditions so that they are able to apply nursing care in a client with a scrotal hernia. Second, Nursing Diagnosis: The results of the study conducted by the author on Mr. F with post-scrotal hernia surgery found acute pain associated with physical injury agents marked by post-operative herniaectomy. High risk of infection associated with invasive procedures characterized by surgery. Impaired physical mobility associated with pain is characterized by decreased range of motion of the patient. Third, Nursing Intervention: The results of data analysis obtained from the results of the study on Mr. F with a postoperative scrotal hernia case, the author has planned nursing actions. The action plan that has been prepared has been adapted to each problem experienced by the client. The focus on this part of the intervention is tailored to the client's circumstances and based on priority diagnoses. Fourth, Nursing Implementation: Nursing implementation carried out by the author to the client also fosters a trusting relationship with the client's family to make it easier to implement the action plan. Fifth, Evaluation: The last step is the evaluation stage which has been carried out optimally and systematically. The evaluation that has been carried out by the author from October 29 to October 31 from the evaluation of the 3 problems obtained by the author can be resolved according to the criteria for the results of each nursing problem.
